Solutions:

Cause:

Project Loaded Families included one or more expected Hanger Families but not the complete list

The Hanger Button checks to see if families need to be added to this project.  If it finds one of the expected families already in your project, it assumes you have all that you want so no more are added.
Solution:

Find the Hanger families for the appropriate vesion of Revit and drag them into your project.

Those hanger families can be found in:

C:\HES\Hydratec for Revit\Data\Families\20xx\Hangers\     where xx = 18,19,20,21  etc. for the Revit version
