Solutions:

Cause:

Bad TOS defline block(s) in the drawing or an object drawn in the DEFLINESSTRUCTURE layer (eg. Line, circle, rectangle, etc).

Solution:

For now, using trial and error, find the bad TOS defline(s) and re-insert them. Also check for lines or circles drawn in the DeflinesStructure layer. In this case, it was easy to spot as the TOS text was not at the endpoint of the blue/magenta defline, it was within the defline (eg. 10' from the end)
